๐ค 88% of enterprise AI agent pilots never make it to production. ๐ค
Almost none of them die because the model was bad.
Forrester's 2026 look at agentic AI adoption went past the headline failure rate and asked why. The answer will sound familiar to anyone who has actually run one of these pilots.
Evaluation gaps: 64% of failed cases.
Governance friction: 57%.
Model reliability: 51%, dead last on the list of blockers.
Even deployments that reach production and still lose money follow the same pattern. 41% trace back to success criteria nobody defined before launch. If no one agreed in advance what "working" meant, no one could tell when it stopped.
Median time-to-value across live deployments: 5.1 months. Sales agents pay back in 3.4. Finance and ops agents take closer to 8.9. Different agents, different clocks, all of them still need someone accountable for hitting theirs.
The 12% that scale share a short list of traits: a named owner, a scoped success metric, automated evaluation running before anything touches production, and the willingness to roll back a bad version without treating it as a verdict on AI itself.
None of that is a model problem. It's a project management problem wearing an AI costume.
