Anyone can chase a result. A runner came in last month convinced his Achilles was “basically healed.” Pain was down, he’d been icing it, taking two weeks off here and there whenever it flared. Made sense to him.
Except when we loaded it, it barely tolerated anything. What he had wasn’t a reactive tendon calming down — it was a degenerative tendon that had been quietly losing capacity for over a year. The rest he kept giving it wasn’t helping. It was the thing making it worse.
This is the part almost nobody explains: tendinopathy isn’t one condition, it’s three different stages, and they need opposite treatment. A reactive tendon calms down with relative rest and reduced load. A degenerative tendon needs the opposite — months of progressive strength work, because time off just makes it weaker without making it less painful.
Same diagnosis on paper. Completely different plan.
If you’ve been managing a tendon issue for months (or years) and it’s not really moving, the stage — not the pain level — is probably why.

Here’s why “just give it rest” was the wrong call for him — and might be the wrong call for you too.
Tendinopathy moves through three stages, and each one is basically a different injury wearing the same name.
Reactive is a tendon that’s swollen, not damaged — it puffed up in response to a load spike. It’s irritated from too much load beyond what it could handle, worse at the start of activity, better once warm. This one actually does respond to relative rest and reduced load. 6 to 12 weeks, usually.
Disrepair is a tendon trying to heal and laying down the wrong kind of tissue. Pain stops settling between sessions. Still reversible — but it needs heavy, slow resistance for months, not time off.
Degenerative is the one that gets missed. Parts of the tendon are no longer functional tendon tissue. It’s often the quietest of the three and the closest to rupture. Rest doesn’t calm it down — it just makes it weaker while the pain stays exactly the same.
Same tendon. Three completely different problems. The stage decides the plan — not how bad it hurts.

Most runners think they only have two options when pain shows up mid-training-block: push through it, or shut it down completely.
There’s usually a third option.
Here’s what that looks like:
→ Video running analysis and a functional assessment to find the actual mechanical fault — not just the sore spot
→ Diagnostic ultrasound when the injury or your race timeline calls for a closer look
→ Targeted treatment — PRP, shockwave, dry needling, or soft tissue work — aimed at the root cause
→ Training adjustments, not training shutdowns, so your fitness keeps building while you heal
→ Structured strength work to fix the weak link that caused the problem in the first place
The goal isn’t just symptom relief. It’s getting you to the start line healthy, with the fitness you worked for still intact.
Achilles tendinopathy, IT band syndrome, plantar fasciitis, runner’s knee, hip flexor pain, shin splints — the earlier these get a real look, the more options you have.

PRP is basically meal-prepping — but with your own blood. And just like meal prep, doing it half-heartedly gets you a sad, underwhelming result.
Here’s the thing nobody tells you: not all PRP is dosed, guided, or followed up the same way. Some clinics draw a small amount of blood, guess at the target, inject, and send you home. Congratulations, you paid for a fancy version of nothing.

Nothing beats starting the weekend with the St. Pete fitness community. Dr. John, Dr. Alex, Dr. Dennis, and Dr. Brian set up shop at F45 Training St Petersburg — diagnostic ultrasound, spot treatments, adjustments, and injury screens for everyone who showed up ready to work on a Saturday morning.

🎂 Happy Birthday to the One and Only Dr. Dennis Colón, DPT!! 🎉

Throwback to 2020: Dennis joins as a student intern with Dr. Brian — back when the “clinic” was Impact Sports Performance and Rehab,. Alliance was in the works, the world was in the middle of COVID, and we were averaging a whopping 8 visits a week. 😅
Humble Beginnings: He was there from the beginning, when Dr. Brian had this upstairs nook! This was after we took down the black soundproofing and blue cloth curtains on the walls. 🤣
Shoutout to Full Torque Fitness for making the way possible and giving Brian a place to start!!
Dennis working on his craft! I (Brian) was new to this whole business thing, and Dennis was needed this whole treating thing. We had plenty of time to talk about treating, being a PT, marketing, and even the small stuff like hand placement and exercise selection.
Even as a student, it was clear that Dennis was going to be a great PT!
And at this point who knew that they would be at each other‘s weddings over the next 5-6 years!

Why does a “minor” ache sometimes turn into months of avoiding the things you love?
When a joint or muscle is injured, the body’s first response is protective: it guards the area, limiting how much you use it. That’s normal and useful — for a little while.
The problem starts when guarding becomes the long-term plan.
⋅ Other muscles compensate to avoid the painful area
⋅ Compensation patterns create new areas of strain
⋅ The under-used tissue gradually loses capacity
⋅ Reduced capacity means the original area feels even less able to handle load
This is why “just resting it” can quietly turn a minor issue into a bigger one. It’s not that rest is wrong — it’s that rest without a plan to rebuild capacity often just delays the real work.
The fix isn’t more waiting. It’s addressing what’s driving the guarding, then rebuilding strength through the range you actually need for the activities you care about — golf, pickleball, skiing, or just getting through the day without thinking about it.
The earlier that process starts, the shorter it usually is.
