Safety Isn’t About Perfection—It’s About Closing the Holes.

The Swiss Cheese Model reminds us that incidents rarely happen because of a single mistake. Instead, they occur when multiple weaknesses—or “holes”—in our defenses line up, allowing hazards to pass through every layer of protection.

On the night of December 2–3, 1984, the Bhopal Disaster became one of the world's worst industrial accidents. A release of toxic methyl isocyanate (MIC) gas exposed hundreds of thousands of people, resulting in devastating loss of life and long-term health impacts.
