Just because the catheter was already there doesn’t mean the additional imaging should automatically be billed.

When renal, abdominal, or lower-extremity angiography is performed during a cardiac procedure, auditors should look beyond convenience. The documentation must support a separate clinical reason for the imaging, a complete diagnostic study, and findings that influenced the patient’s care.

The opportunity to perform a service does not establish medical necessity.

Are we teaching physicians how to document their work—or how to pass an audit?

There is an important difference.

Physician education can easily become a checklist:

“Document the status of the condition.”
“Mention prescription drug management.”
“Add your review of the test.”
“Include this statement.”

Each instruction may begin with a legitimate purpose. But when providers learn the phrase instead of the concept, documentation becomes less about communicating the encounter and more about satisfying perceived audit criteria.

Here is a question every physician educator should consider:

Would I still recommend this documentation if there were no E/M level attached to it?

If the information helps another reader understand the patient’s condition, decision making, management, or reason for the service, we are teaching meaningful documentation.

If the answer is simply, “because the auditor wants to see it,” it may be time to reconsider the education.

A well-documented record should not look like it was written for an auditor. It should tell the story of the encounter clearly enough that an auditor can understand what happened.
