Meet Colleen, PT, DPT, CLT, MBA 🩵

We're so excited to welcome Colleen to the Neuro Nation team! She has spent more than ten years helping people get their movement and their independence back. She's worked in multiple settings and she's especially good with the complicated cases: the patients whose recovery doesn't follow a straight line.

She's also a Certified Lymphedema Therapist (CLT), which means she can treat the swelling that often comes along with a stroke and limited mobility. She will be leading our Nushu shoe consults for freezing in Parkinson's Disease.

Here is a number nobody explains at discharge: the authorized therapy visit count. It is set by the insurance plan, not by what your mom's brain needs to relearn how to walk. When the visits run out and progress looks slow that gets filed as "her limit." A lot of the time it is the limit of the insurance authorization. That gap is the reason we built our programs, in-home PT and OT program in the Triangle, NC and Jacksonville, FL.

With a stroke there's a start date. Parkinson's Disease doesn't give you one.

It gives you years of small things: a tremor, slowing down, smaller handwriting, or a voice you keep asking him to turn up.

You noticed first because you're closest to it.

A hemi-walker is often read as the end of the road. It is closer to the start of one. When we train in the home, in the Triangle or in Jacksonville, an assistive device is what lets someone practice hallway walking safely while the therapist fades the assistance required week over week. When a family hears "progressive disease," every loss can start to feel like the disease just doing what it does. Some of it is, but some of it is a year of moving less sitting on top of the diagnosis, which is the part skilled therapy can work. Naming which is which is imporant. Picking something up off a chair looks like nothing. After a stroke, it is an important skill.

Watch the hips hinge back and the affected side sharing the load. A controlled lift, and a slow return.

This is how he reaches for what he dropped, without a fall.

The walker is right there and the goal is needing it less.

"She's plateaued."

When someone says that about your mom, it lands like a fact about her. A lot of the time, it's a fact about the program she was on.

A plateau is what shows up when the input stops changing. The same exercises, twice a week for only thirty minutes is not going to cut it. The body adapts to a steady input and then holds steady.

People read that holding pattern as the limit of what she can do. Often it's the limit of what that type of program can produce.

Change the input and the line tends to move again. Try harder tasks or more repetition. Work that follows her into her house instead of ending at the clinic door.

So before you file "plateau" under "this is as far as she goes" it's worth asking what a bigger dose would show.

A plateau on a small program tells you very little about your mom on a harder one.

Getting out of bed looks like nothing. Until it is what decides whether someone can be alone in the morning.

This is the rep that never makes the highlight reel. Lowering himself back to the bed with control. Then rolling to his side. Pushing up. Finding the edge and sitting up on his own.

The brain relearns that sequence the same way it relearns everything. The right rep, done often enough.

We practice it where it happens. His bedroom and his first move of the day. A transfer trained on a clinic mat table is a different problem than the same transfer on a soft mattress at 6 a.m.

Because independence does not begin in a clinic. It begins the second he wakes up.

This is the rep almost nobody films.

Sitting down and standing back up.

It looks small. It is the movement that decides whether someone can get off the couch, out of a chair, out of the car, or off the toilet without a hand to pull them. Sit-to-stand is the gateway to most of what independence consists of.

The brain relearns it the same way it relearns everything. The right rep, often enough.

Slower than anyone wants. And it is the difference between waiting for help and standing up on your own.
