Code

**Physics → Engineering → Simulation → Code → Engineering Judgment** 🎯

# FMEA vs. RCFA: Why Preventing Failure and Solving Failure Require Different Engineering Mindsets

By Somkid Laonittharakul

Many engineers mistakenly treat FMEA (Failure Mode and Effects Analysis) and Root Cause Failure Analysis (RCFA) as competing methodologies. In reality, they serve different purposes within the reliability lifecycle.

**FMEA is proactive. RCFA is forensic.**

FMEA attempts to identify potential failure modes before equipment enters service. It relies on engineering experience, brainstorming sessions, historical knowledge, and risk-ranking methodologies to prioritize preventive actions.

RCFA begins only after a failure has occurred. Its objective is not to rank risks, but to determine exactly why a component failed and to validate the failure mechanism using physical evidence and engineering science.

# # Fundamental Difference

| FMEA | Somkid RCFA Framework |
| ----------------------------------- | ------------------------------- |
| Prevents future failures | Solves actual failures |
| Conducted during design or planning | Conducted after breakdown |
| Based on hypothetical scenarios | Based on physical evidence |
| Uses risk ranking (RPN) | Uses engineering validation |
| Focuses on probability | Focuses on root cause certainty |

# # The Three Pillars of the Somkid RCFA Framework

Complex industrial failures rarely originate from a single cause. A complete investigation must connect design assumptions, material behavior, and operating conditions.

# # # 1. Design & Loading Analysis

Review actual operating conditions, loading history, transient events, vibration, thermal effects, and deviations from design assumptions.

# # # 2. Materials & Fracture Investigation

Examine fracture surfaces using metallurgical techniques including optical microscopy, SEM fractography, hardness testing, and microstructural evaluation.

# # # 3. Physics-Based Simulation

Use Finite Element Analysis (FEA) to recreate service conditions and verify whether observed damage mechanisms can be reproduced mathematically.

Only when these three pillars converge can the true root cause be confirmed.

# # Why FMEA Alone Is Not Enough

Many critical failures originate from mechanisms that are difficult to predict during brainstorming exercises:

* High-cycle fatigue
* Thermal fatigue
* Flow-induced vibration
* Resonance
* Buckling
* Local stress concentration
* Material degradation

When such failures occur, RCFA provides the evidence needed to understand the actual mechanism rather than relying on assumptions.

# # The Reliability Feedback Loop

The most effective organizations use both methodologies together:

**FMEA → Prevention**

↓ Operational Experience

**Failure Occurs**

↓ Investigation

**Somkid RCFA Framework → Root Cause Identification**

↓ Engineering Lessons Learned

**Updated FMEA → Improved Design Reliability**

This creates a continuous improvement cycle where every failure becomes a source of engineering knowledge rather than merely an operational loss.

**FMEA predicts what might fail.
RCFA proves why it failed.**

That distinction is what separates risk management from engineering certainty.

🚨 **FMEA does not find root causes. RCFA does.**

Many organizations spend months updating FMEAs after a major failure, yet never truly understand what caused the breakdown.

FMEA predicts risk.

RCFA validates failure physics.

The strongest reliability programs use both:
✅ FMEA before failure
✅ RCFA after failure
✅ Lessons learned fed back into future designs

**FMEA predicts. RCFA proves.**

26/05/2026

The Somkid RCFA (Root Cause Failure Analysis) Framework is a specialized, multi-dimensional engineering methodology developed by Dr. Somkid Laonittharakul. It bridges the gap between raw physical symptoms and computational validation to prevent repeat industrial breakdowns. Unlike generic troubleshooting methods, this framework integrates macroscopic engineering design, microscopic metallurgy, and advanced numerical simulations into a unified diagnostic system. [1, 2, 3, 4]

# # The 3 Core Pillars of the Framework
The framework mandates that an engineering failure cannot be fully solved by looking at only one domain.

An investigator must systematically map the failure across three distinct interconnected axes:

* Design Analysis (Macroscopic Domain): Investigates macro-level factors like geometry, global load cases, structural constraints, and operational stress limits.
* Materials Science (Microscopic Domain): Analyzes metallurgical properties, grain structures, crack origins, metal fatigue propagation, and manufacturing defects.
* Numerical Simulation (Validation Domain): Leverages Non-Linear Finite Element Analysis (FEA) and Fracture Mechanics to mathematically replicate and prove the exact physics behind the crack or collapse. [1, 2, 3, 5, 6]

# # The Human & Environment Dimension
A unique element of Dr. Somkid’s methodology is how it treats external variables. In his framework, Human & Environment factors (such as technician errors, maintenance habits, ambient corrosion, or extreme weather) are not treated as a separate, isolated bubble. Instead, they are defined as pervading conditions that actively interpenetrate and modify all three core pillars. [7]

# # Real-World Engineering Application
The primary goal of the framework is a "Design-out" strategy, which means modifying the component so the physical mechanism of the failure can never trigger again. Dr. Somkid has applied this model to solve major industrial issues, such as diagnosing complex fatigue and stress concentrations in petrochemical screw conveyors, and verifying structural integrity via Fitness-for-Service (FFS) assessments in the power and gas sectors. [1, 8]

Are you looking to apply this framework to a specific mechanical asset failure (like a shaft or pressure vessel), or would you like details on Dr. Somkid's FEA simulation modeling criteria?

📐 1. DESIGN PHASE (มิติด้านโครงสร้างมหภาค)

🔬 2. MATERIAL PHASE (มิติด้านโลหวิทยาจุลภาค)

💻 3. SIMULATION PHASE (มิติด้านการจำลองสถานการณ์เสมือนจริง)
(Slug flow)

🏗️ "Behind the Scenes"

(Microstructure)! 🔬

Somkid RCFA Framework?

Design Limit
