Requirements for PEMCOR 3on3 at ATC:

1. Photocopy of proof of birth - NSO Certified Birth Certificate/Passport
2. Photocopy of school ID/Valid ID
3. Accomplished waiver form
4. Accomplished photo-release form
5. Accomplished registration form (per team)
6. Registration Fee: P1,000.00
7. Bring uniform

ATC 3 on 3 Slam Jam presented by PEMCOR:

When: May 27 (Friday)
Where: Activity Hall, Alabang Town Center

Divisions:
Men's 16-under (born 2000)
Ladies' 17-under (born 1999)

Pre-Registration (ongoing):
9-10am- Men's Division
12-1pm- Ladies' Division

Games Start:
10am- Men's Division
1pm- Ladies' Division

Uniform: bring your own light and dark jerseys

Requirements:
1. proof of birth - photocopy of birth certificate/passport
2. school ID/valid ID
3. accomplished information & waiver form of minors
4. accomplished registration form
5. registration fee of P800.00/team
