Persistent collarbone, upper chest or anterior shoulder tension?
One often overlooked structure may be the subclavius muscle - a small stabilising muscle located beneath the clavicle.
The subclavius plays an important role in:
• stabilising the clavicle during shoulder movement
• supporting sternoclavicular joint mechanics
• protecting nearby neurovascular structures
This area may become irritated or overloaded due to:
* blunt trauma during contact sports
* repetitive overhead activity
* prolonged poor posture
* altered shoulder mechanics
* clavicular injury
Potential symptoms may include:
• collarbone pain or tightness
• anterior shoulder discomfort
• upper chest tension
• pain with overhead movement
• local tenderness beneath the clavicle
However, symptoms are rarely caused by one structure alone.
This is why assessment should also consider:
• thoracic posture
• scapular control
• rib mechanics
• cervical contribution
• movement and loading patterns
Persistent symptoms are often a sign that the body is no longer managing load efficiently.

Lower back pain and hip function are closely linked.
The hips and lumbar spine work together to create and transfer movement during walking, running, lifting and squatting.
The spine is naturally designed to absorb and manage load. However, when hip mobility, strength or control are reduced, movement strategies and load distribution can change.
Over time, this may contribute to increased stress sensitivity in the lower back and surrounding tissues.
Common findings associated with lower back pain can include:
• reduced hip mobility
• glute weakness
• altered pelvic control
• reduced single leg stability
This is why rehabilitation should look beyond the painful area alone.
Understanding how the hips, pelvis and spine work together is essential for improving movement quality, load tolerance and long-term outcomes.

Not all shoulder pain comes from the same structure.
Different joints and tissues become irritated based on trauma, load demands and movement patterns.
AC Joint (top of shoulder)
Often irritated after falls, tackles or direct compression.
Common symptoms:
• Pain at the top of the shoulder
• Pain with cross-body movements
• Pain during pressing exercises
• Discomfort lying on that side
SC Joint (front of chest/clavicle)
Can be affected by higher-force trauma or collision injuries.
Common symptoms:
• Pain near the collarbone/chest
• Pain with pushing movements
• Pain with heavy lifting
• Visible swelling in some cases
Subacromial Space (outer shoulder)
Often irritated through repetitive overhead loading or poor movement mechanics.
Common symptoms:
• Pain lifting overhead
• Pain lowering the arm
• Weakness during overhead tasks
