What is a learning disability annual health check?

An annual health check is an appointment when you meet your GP to
◾Check everything about your health
◾Help you stay healthy

You can have an annual health check if you are over 18 years old and your GP knows you have a learning disability.

There are things you can do to get ready for your appointment - https://performanceandimprovement.nhs.wales/functions/quality-safety-and-improvement/improvement/our-work/learning-disability/resourses-learning-disability-health-improvement-programme/annual-health-check/2026-documents/getting-ready-for-your-annual-health-checkpdf/

Here is more information to help you get ready your Annual Health Check appointment https://performanceandimprovement.nhs.wales/functions/quality-safety-and-improvement/improvement/our-work/learning-disability/resourses-learning-disability-health-improvement-programme/annual-health-check/2026-documents/going-to-your-annual-health-check-appointment/

It is a good idea to fill in a health profile before you go to your annual health check appointment.

A health profile is a booklet where you can keep information about you and your health and any support you need to make health appointments easier.

A health profile:

◾Tell us about your needs
◾Help our staff make reasonable adjustments for you
◾Receive patient-centred care at the right time

You can get a Health Profile at www.improvementcymru.net/health-profile
