Not all arthritis is the same, and that matters for how we approach movement. 🦴
Osteoarthritis (OA) involves the gradual breakdown of the cartilage cushioning the joint, alongside low-grade inflammation within the joint itself. Movement here is often about building support around the joint, with steady, controlled work that helps the joint feel more stable and function more efficiently. Also, gentle mobility work helps maintain synovial fluid exchange that keeps the joint fed, as cartilage has no direct blood supply and relies on joint movement to stay nourished.
Autoimmune arthritis (like rheumatoid arthritis or ankylosing spondylitis) is different, the immune system mistakenly attacks the joint lining itself, causing more pronounced, systemic inflammation. Appropriately dosed movement is now understood to support joint function without accelerating damage, alongside broader benefits for bone density and cardiovascular health, both of which can be affected by autoimmune arthritis and its management. Here, movement needs to be more responsive: gentle on flare days, building capacity on good days, always working with what the joint is telling you.
This is where personalised Clinical Pilates really comes into its own. Rather than following a standard class, every one-to-one session is adapted to your type of arthritis, your symptoms and how your body feels that day, helping you move with greater confidence, build strength safely and support your joints without unnecessary strain.
Living with arthritis? Your osteopath, or rheumatologist, is always the best starting point for a plan tailored to you.

Autoimmune conditions affecting the joints are widely associated with inflammation, but inflammation doesn’t just cause pain. Over time, it can also compromise the quality and resilience of connective tissue itself.
In conditions such as:
- Rheumatoid arthritis
- Lupus (SLE)
- Psoriatic arthritis
- Ankylosing spondylitis (where enthesitis and structural remodelling play a distinct role)
- Hashimoto’s thyroiditis (where hypothyroidism can secondarily impair connective tissue quality and contribute to joint and muscle pain)
Chronic inflammatory activity can affect:
- Collagen integrity and turnover
- Ligament and tendon function
- Overall joint stability
This creates a secondary cycle worth addressing:
instability → mechanical stress → persistent pain → further inflammatory load
Clinical implication:
Managing inflammation is essential, but it may not fully resolve symptoms if structural and mechanical factors are left unaddressed.
A more comprehensive approach may also include:
🌀 Supporting joint mechanics and stability
🤸‍♀️ Progressive strength and loading programmes
🥗 Addressing systemic factors that influence tissue repair
In autoimmune conditions, pain is rarely purely inflammatory; it often has a structural component that develops alongside it.
A comprehensive approach starts with the right conversation. Book your complimentary 15-minute consultation today. Link in bio.

Last Friday, Matthew and I met with our wonderful Pilates instructors, Gokben, Melissa, Erin and Beverley, for a lovely dinner together at Brother Marcus.
Running a clinic is not just about treatments and sessions. It’s about the people behind it, the conversations, the shared ideas, the laughter, and the care we all bring to the work we do every day.
We feel incredibly grateful to have such thoughtful, talented women supporting our clients and bringing so much warmth to the studio.
A very special evening with our team, and many more Wellthy socials to come 🥂
