Tibial rotation is one of the most overlooked movements when it comes to knee, ankle and hip function.

Your tibia isn’t designed to only bend forwards and backwards. It also internally and externally rotates as you run, land, squat and change direction.

Research shows:

• Tibial rotation is mechanically coupled with movements at the foot and knee, meaning altered rotational mechanics can change how forces are transferred through the lower limb (Levens et al., 1948).

• Excessive or poorly controlled tibial rotation can influence knee joint mechanics and ACL loading, particularly during movements involving rotation and valgus (Meyer and Haut, 2008).

• Foot mechanics can influence tibial rotation further up the kinetic chain, showing why the ankle, tibia, knee and hip shouldn’t be assessed independently (Nester et al., 2003).

Think of the chain:

Foot position → tibial rotation → knee tracking → femoral/hip control → force absorption.

If you don’t have enough rotation, or you can’t control the rotation you have, your body may find that movement somewhere else.

That’s why simply strengthening the painful knee or stretching the ankle doesn’t always solve the problem.

And tibial rotation isn’t the single cause of ACL tears, ankle sprains or hip and knee pain. Injuries are multifactorial. It’s one movement variable that needs to be assessed within the whole system.

Don’t just look at where it hurts.
Look at how the entire chain moves.

Speed and explosiveness are not built by simply moving faster or jumping more.

Your hip internal rotation could be one of the most overlooked movements in your lower body.

Your hip needs rotation when you run, cut, land and change direction. When that movement is restricted or poorly controlled, another part of the chain may have to compensate.

Research shows:

• Athletes with hip and groin pain demonstrate reduced hip internal rotation compared with pain-free athletes (Mosler et al., 2015). (PubMed⁠)

• Reduced hip internal rotation has also been associated with worse function in people with lower back pain (Prather et al., 2017). (PubMed⁠)

• Hip rotation is also linked with knee mechanics during landing and cutting, including knee valgus and rotational loading relevant to ACL injury mechanisms (Hodel et al., 2025). (PubMed⁠)

Think of the chain:

Hip rotation → pelvic control → femur position → knee control → foot placement.

But this is important: lack of hip internal rotation isn’t automatically the “root cause” of back, groin, knee or ACL injuries. Injury is multifactorial. You need enough mobility and the strength and control to use it.

Don’t just stretch your hips. Own the rotation under load.

Your pain might not be coming from where you feel it.

Marcel’s hip had healed “on paper”, but heavy squats and leg presses were still causing problems.

His MOT assessment exposed hip restrictions, pelvic compensation and unnecessary pressure through his knee.

That is why chasing symptoms was never going to fix it.

His hip injury had “healed”, but years later, Marcel still could not squat or leg press properly without pain.

The real issue was not just where he felt the pain.

His MOT assessment exposed:

• A clear difference between his left and right hip
• Restricted hip mobility
• Pelvic compensation
• Extra pressure being pushed into his knee

Painkillers, stretching and quick fixes may reduce symptoms. They do not automatically correct the movement problem driving them.

Pain keeps returning because the painful area is not always the source of the problem.

If you cannot sprint at full speed, change direction or absorb force without pain creeping in, more stretching will not prepare you for the season.

The MOT assesses your whole movement system, including:

• Sprint mechanics
• Change of direction
• Mobility and joint control
• Strength and force absorption

Stop managing the same symptoms. Find what is placing excess stress on your body and address it before pre-season.
