We commissioned Richard Rosewarne, one of the country’s leading care assessment experts to design our care assessment toolkit. Our assessment process takes an individual approach to quality of life and determines what really matters for people seeking aged care services supports. The Linkage Care assessment toolkit aims to:
 Understand the customer’s query from their perspective;
 Use a conversational style that listens to and values the customer input;
 Be based on objective information;
 Assist the customer to identify their needs, goals and priorities;
 Have clear outcomes that the customer can understand and modify;
 Determine the services that will best fit the customer’s personal situation;
 Provide guidelines and training to support the healthcare professionals that are part of the assessment process. The Linkage Care assessment process covers a broad range of topics that will identify how best to support each person.
