Alzheimer’s Begins Decades Before Memory Loss.
When does Alzheimer’s begin? Most people would probably point to the moment memory starts to noticeably decline. The research is pointing much earlier.
A 2025 study published in PNAS analyzed data from more than 19,000 people and identified a striking timeline in brain aging.
Around 44: Early changes in brain-network stability may already be emerging.
Around 67: Network destabilization reaches its most rapid period of change.
70+: This is when the cognitive symptoms we traditionally associate with aging and dementia may become increasingly recognizable.
That creates an important distinction: The beginning of biological change and the beginning of noticeable symptoms are not necessarily the same moment. And researchers are increasingly examining metabolism as part of the reason why.
The 2025 study identified neuronal insulin resistance as an important factor associated with this transition in brain-network stability. This connection isn't entirely new.
In a Mayo Clinic study published in Diabetes, researchers found that 81% of patients with Alzheimer's disease had either impaired fasting glucose or type 2 diabetes, compared with 42% of age-matched controls.
That doesn't mean diabetes and Alzheimer's are the same disease. It does mean the relationship between metabolic health and brain health deserves our attention much earlier in life.
Insulin sensitivity. Glucose regulation. Visceral fat. Muscle mass. Inflammation.
These aren't simply numbers or health concerns belonging to different parts of the body. Together, they help shape the metabolic environment in which the brain has to function for decades.
If meaningful changes can begin in midlife, then waiting until someone's memory begins declining means starting the conversation much later than we could have.

Your Body Is Already Telling the Story. You Just Need to Know Where to Look.

Take fasting glucose, for example. Most people have been taught to ask one question: “Is my glucose normal?” That’s an important question. But it isn’t the only one.
In the study we’re highlighting this week, fasting glucose and hemoglobin A1c were the metabolic markers most closely associated with long-term brain aging. But even that finding doesn’t mean glucose should be interpreted alone.
Visceral fat is metabolically active. It influences liver function, insulin resistance, glucose regulation, and inflammation. That means a change in one part of the system can begin showing up across several biomarkers long before any single number tells the whole story.
Glucose doesn’t exist in isolation. Neither does insulin. Neither do triglycerides, HDL cholesterol, liver enzymes, or inflammatory markers. They’re connected.
That’s why, at Opti, we rarely focus on one biomarker by itself. We ask how these biomarkers are moving together, what metabolic systems they may be reflecting, and—most importantly—which direction the overall pattern suggests your metabolism is moving.

Two People Can Lose the Same Weight...and Have Completely Different Outcomes
If two people each lose 20 pounds, would you assume they became equally healthy? Most of us probably would. After all, the scale says they achieved the same result.
But your biology doesn't measure success the way a bathroom scale does. Two people can lose the same amount of weight and still experience very different biological outcomes. The scale can tell you how much weight changed. It can't tell you what changed beneath the surface.
One person may lose more subcutaneous fat—the fat just beneath the skin that you can pinch. Another may lose more visceral fat—the deeper fat stored around the liver, pancreas, intestines, and other organs. And that distinction matters.
In the study Dr. Brotzman shared this week, losing weight itself did not predict healthier brain aging. Losing visceral fat did.
The researchers found that sustained visceral-fat loss was associated with less brain atrophy and better cognitive function over time, while changes in body weight and BMI were not associated with those same outcomes.
Why would visceral fat matter so much? Because visceral fat isn't simply stored energy. It's metabolically active tissue. It releases inflammatory signaling molecules into the bloodstream, and because blood leaving visceral fat drains directly to the liver through the portal vein, those signals reach one of the body's most important metabolic organs first and at their highest concentration.
That's one of the reasons visceral fat sits upstream of insulin resistance and other metabolic dysfunction. The scale sees 20 pounds. Biology sees what changed.
That's why, at Opti, we spend less time asking only whether the number went down and more time asking what changed beneath it. Is glucose regulation improving? Are metabolic risk signals moving in a healthier direction? And what story are those changes telling when we look at them together?

Your body has never seen a diagnostic guideline. It doesn't know what fasting glucose qualifies as diabetes. It doesn't know where a medical society chose to draw a diagnostic line. It simply responds to the environment it's given, adapting to the metabolic signals it experiences every day.

Your Blood Work Is Telling a Story
Imagine picking up a novel, opening to a random page, reading a single paragraph, and deciding you know the entire story. Most of us would never do that.
We understand that every chapter builds on the one before it, and that the meaning often comes from the relationships between the characters, not from one paragraph taken in isolation. Your blood work works much the same way.
Every biomarker tells part of the story. But the real insight often comes from understanding how those biomarkers relate to one another.
For decades, much of medicine has focused on asking questions like:
- Is my cholesterol normal?
- Is my blood sugar normal?
- Is this lab value in range?

Imagine taking your car to a mechanic because smoke is coming from under the hood.
The mechanic measures how much smoke is coming out, writes the number down, and tells you to come back in six months.
Most of us would probably ask... "That's helpful...but what's causing the smoke?"
