The biggest comparison yet ran to seventy-one studies and 523 effect sizes, and it found group and one-to-one training both effective. Your client did their first proper leg session on Monday. By Wednesday they cannot get down the stairs without holding the rail, and they are telling you it is lactic acid.
It is not. Lactate clears from the blood within about an hour. Soreness peaks a day or two later. When researchers measured blood flow and lactate removal during massage, massage impaired both. The soreness is mostly a sensory system turning the volume up, not a tissue problem waiting to be flushed out.
Across 27 trials massage reliably reduces how sore somebody reports feeling, pooled at a standardised mean difference of −0.62, and does nothing measurable for strength, jump, sprint, endurance or creatine kinase. Light movement is the opposite. It barely touches soreness but ranked first of everything tested for restoring jump performance at 24 hours. Massage is the key when how they feel is the goal. Use light movement when the next session is the goal.
Our new article works through 29 systematic reviews and 863 trials to set out where massage belongs, where active recovery belongs, why the head-to-head trial everybody quotes does not actually exist, what the placebo evidence says, and the one thing on the list that makes a client less sore next time rather than just more comfortable today.
