1. We start with a conversation
We learn about the participant’s current situation, preferred location, goals, routines and support requirements.
2. We understand the person
This includes communication preferences, daily living needs, interests, personal-care requirements and what helps the person feel settled and understood.
3. We explore compatibility
Our team considers the participant’s needs, preferences and shared-living dynamics to determine whether one of CCA’s current homes may be suitable.
4. We plan the transition
Where a suitable match is identified, CCA works collaboratively with the participant, family, support coordinator and broader professional team to plan the next steps.
